The Marshall islands, once the testing grounds for 20th-century weapons of mass destruction, remain perversely beautiful. But the infested waters become the battleground for looting pirates and sec men in still-functional navy PT boats, all driven by greed and madness to plunder the predark caches of science and technology hidden in the islands. Ryan Cawdor awakens from a mat-trans jump to inexplicably find himself in a version of his past (just after the events of Deathlands 45: Starfall). Ryan's son Dean is back, Krysty isn’t pregnant yet, and Ryan, who is the only one who feels the pull of the timeline split, is trying to keep it together. The companions are in immediate need of medical supplies and get caught up in a clash between warring barons and a traveling gypsy caravan and circus. Someone ambushes and is sabotaging the gypsies and the companions decide to help.

The companions have returned from their time jump back to the year 2001 on a mission to put an end to the malevolent cyborg known as Magus. Despite having narrowly survived that harrowing adventure, Ryan Cawdor’s intrepid team is not about to give up. They renew their pursuit of the augmented madman. But there are disturbing indications that the Deathlands to which they’ve returned may not be the one they remember.

Through a misunderstanding, Doc Tanner is believed to have achieved 246 of such acts.

Ryan and his group make their way to a remote island in hopes of finding brief sanctuary. Instead, they are captured by an isolated tribe of descendants of African slaves from pre-Civil War days. When the tribesmen declare Mildred Wyeth "free" from her white masters, it is a twist of fate that ultimately leads the battle-hardened medic to question where her true loyalties lie. Will she side with Ryan, J.B. Dix, and those with whom she has forged a bond of trust and friendship...or with the people of her own blood?

Stumbling into Restown’s hunting grounds, the companions are welcomed by a flourishing society which stands in counterpoint to Mutieville, a place where enslaved muties are subject to human whims. At the heart of Restown lies an ambitious plan to utilize cryogenic technology to escape the brutal present of the Deathlands and awaken to the glorious future that’s being constructed for the baronial family by their slaves.

Hungry and exhausted, Ryan and his band emerge from a redoubt into a remarkably untouched pre-dark city and uncover a cache of weapons and food. But the group is not alone. Among other interlopers, the city is guarded by huge winged creatures, protecting the grisly secrets of a nearby barony. Holed up inside an old government building, where Ryan's son, Dean, lies near death, Ryan and Krysty must raid the treacherous ville across the desert where a local baron uses human flesh as fertilizer.

Bizarre murders are taking place in a fertile farming community, and the locals are quick to point fingers at Ryan and his companions. But they know another culprit is responsible. A colony of mutants has been driven from its underground home, forced to find sustenance in the light of day. And only human flesh will satisfy their hunger. Caught between a rock and a horde of hungry cannibals, Ryan and the companions face an ultimatum — help the cannies reclaim their territory, or risk becoming the next meal.
