Ryan Cawdor awakens from a mat-trans jump to inexplicably find himself in a version of his past (just after the events of Deathlands 45: Starfall). Ryan's son Dean is back, Krysty isn’t pregnant yet, and Ryan, who is the only one who feels the pull of the timeline split, is trying to keep it together. The companions are in immediate need of medical supplies and get caught up in a clash between warring barons and a traveling gypsy caravan and circus. Someone ambushes and is sabotaging the gypsies and the companions decide to help.

Following the events of Deathlands 133: Grim Choices, Ryan Cawdor and his companions find themselves far from their intended destination of the surviving redoubt on the outskirts of what once was the capital of a mighty nation. The gateway they’ve arrived through has planted them in the geographic middle of the Deathlands, and they soon find themselves in a violent struggle between multiple warring factions for the spoils of a legendary stockpile, buried in a former military installation somewhere in the region.

Taken captive on a ship in the former Caribbean, Ryan and his companions must work as part of the crew or perish at the hands of the captain. But the mutant in charge of the vessel is the least of their worries. Each day is a struggle as they face rivalry among the sailors, violent attacks, and deadly storms. Worse, a powerful enemy is hunting the ship to destroy everyone on board. In the epic conclusion to the Cornerstone saga, the sinister forces at work in the troubled ville grow insidiously stronger, and Ryan Cawdor is the only one with first-hand knowledge of the true enemy. The former metropolis is not deserted, however, but densely populated with ravening hordes of barely human creatures.

When the brothers head into the small cow town of Crossville, close by Devil Creek, all they want is to stock up on supplies and rest their weary horses. Instead, they find themselves in the middle of a bloody range war that’s literally split the town down the middle. The warring factions have set up camp on either side of Main Street and the newcomers ride in on a deserted strip of no man’s land where anything - and nothing good - is liable to happen.

Dix takes several arrows before his team manages to extricate themselves, and, while the wounds themselves are not fatal, Dr. Mildred Wyeth worries with good reason that the arrows may have been tainted. J.B. soon shows symptoms of a deadly infection. Their best chance is a journey of several days to where there had once been a hospital when Mildred trained there a century earlier, in the desperate hope of finding the drugs that J.B. needs to survive.

Ryan and his friends become the subjects in a deadly experiment when they're taken captive inside a redoubt at the South Pole. A team of scientists is convinced the Earth must be purified of mutants, and now they have the perfect lab rats to test their powerful bioweapon. Within Antarctica's harsh and unstable conditions, the companions must fight the odds and take down the whitecoats before millions are killed.
