Dealing with the fallout from a difficult case, Welsh Inspector Ian Drake faces a deadly foe as well as counselling for his OCD. When a ferry arrives in port, he knows a killer is on board. Unable to trace the killer after the ship berths, he fears the worse. Tension mounts when two more members of the crew are killed. When Drake’s cousin gets involved and he faces the implications of his father’s treatment for cancer, he struggles to face the challenges of tracking the killer.

The day gets off to a bad start for Montalbano: while trying to break up a fight on Marinella beach, he hits the wrong man and is stopped by the Carabinieri. When he finally gets to the office, the inspector learns about a strange abduction: a woman was abducted, drugged, and then released unharmed a few hours later. A few days later, the same thing happens again, but this time the woman abducted is the niece of Enzo, the owner of Montalbano's favorite trattoria. The only link between the two events is that both women are 30 years old and work in a bank.

Christmas day, and DCI Tom Reynolds receives an alarming call. A mass grave has been discovered on Oileán na Caillte, the island which housed the controversial psychiatric institution St. Christina's. The hospital has been closed for decades, and onsite graves were tragically common. Reynolds thinks his adversarial boss is handing him a cold case to sideline him. But then it transpires another body has been discovered amongst the dead - one of the doctors who went missing from the hospital in mysterious circumstances 40 years ago.
